Objetivos: To evaluate nutritional status of children of rural schools from Pirai do Sul, Parana.
Métodos: A cross-sectional study was performed with 332 children (aged 4-13-years-old, 45.5% [n=151] girls) in rural schools (conventional [n = 3] and multigrade [n = 7]). Body mass index (BMI), neck circunference (NC), waist-to-hip ratio (WHR), waist-to-height ratio (WHtR) and conicity index (C index) were analyzed. ROC analyses were performed to calculate the cut-off points for each variable and their abilities to diagnose children with excess weight (using BMI-for-age >85th percentile).
Resultados: The sample studied represents about 15% of the children enrolled in the entire municipality (considering rural and urban areas). According to BMI z-score, 67% of children were eutrophic, 32% overweight/obese (of which 48% overweight, 35% obese and 17% extremely obese) and 4% lean. Overweight/obesity ranged from 24-34% in conventional schools to 36-70% in multigrade small schools. WHtR performed significantly better as a measure of excess weight than did other variables in girls and boys, respectively: WHtR AUCs = 0.97 and 0.93; NC AUCs = 0.94 and 0.81; C index AUCs = 0.87 and 0.73; and WHR AUCs = 0.82 and 0.64). Our cutoffs for all variables are presented for both sexes.
Conclusões: WHtR provides a simple and effective tool for the diagnosis of overweight and obesity in children. Multigrade education in rural schools requires a discussion that covers not only pedagogical but also structural criteria, such as those related to the impact of school meals quality on the nutritional status of students.
